Can someone give me an example of a Prima Facie Case of Employment Discrimination?

An otherwise qualified employee with a disability requests a job accommodation and it is denied by the employer.

A company in San Francisco with 400 employees do not have any Blacks, Hispanics, Asians or Pacific Islanders. It also has no Jews or gays. All the employees are white, straight and Christian. The odds against that are astronomical UNLESS there is discrimination.
