How to transition off SSDI?

I've been on SSDI for over 10 years now due to long-term HIV infection. Thankfully, with new treatments, my health has improved a great deal and I would love to go back to work.

The problem is that if I go back to work I would lose all my benefits and not be able to afford my medications. My State ADAP program will throw you off faster that Social Security will! And if I manage to make it past the 9-month grace period and come off the SSDI roles, then what happens if I get sick again? It takes a minimum of 6 months and up to 2 years to get SSDI approved again.
Not to mention that fact that the industry in which I once worked doesn't even exist anymore due to technical advancements in the field. So I'd be a 50yo in an entry level position trying to cope with overwhelming expense and busting my butt to make ends meet.
Not that I exactly live like a king on my current income, but I can't find any feasible way to make enough money to support myself if I make a change.
Any thoughts

SSDI has a work program where you can try to work and if you can not succeed at it you will go back on benefits.~
