Why might repeatedly forgiving compayments constitute fraud on the insurance company?

A. Because the doctor is in effect misstating the true fee, resulting on overpayment
B. Because patients are legally obligated to pay the copayment
C. Because it shows up as a false tax deduction on the doctor's income tax return or
D. Because it defrauds the patient who has the contract with the insurance company

A. The patients insurance cover pays the doctor, so if the doctor is falsifying the number of his/her patients for financial gain, then that would equate to an act of fraud on the insurance company.
