How to report workman's comp fraud??

How would you report workman's comp fraud if you do not know the company the person worked for or the insurance company the claim would have been filed with.

The only information known is name and address.

And no need to suggest turning the information into the cops because that would be a dead end as they prove to be of no service in our area.

How about the workers compensation board?
I think most provinces and states have one... just google your location with "workers compensation board" and it should tell you which numbers to call.
