Well they need your S.S number, so you parents could have gotten you one when you were a kid. You would never need to get a physical which alot of people do ,and if you are over the age of 18 and never had one they can'nt get one with out you knowing because you need to get a physical.(unless some one Completely stole you identidy)
If you think your parent may have gotten you one when you were a kid just find out what company and they should beable to find it by your last name if you dont have a pl number.

Nobody can insure your life without your signature. If you think someone has forged your signature contact your state's department of insurance and ask them how to find out. You'll probably have to send a letter to all the big-name insurance companies.
