Some states have passed a law that the person who has the policy taken out on is to be notified. See if your state is one that requires this.

Yes, they can. I found this out a couple years ago.

usually your work does.. minimal as it is.. but they let you know.. as for someone else .. not as far as i can think of.. unless youve had your identity stolen..
