No health insurance. Do I have a rights to receive itemized bill?

I live in Colorado. Does it say somewhere that patient has rights to receive itemized bill from their doctors, hospitals? Psysician hired by hospital in emergency room billed my separatly from hospital. Psysician bill is not itemized ,just shows couple of codes and amount to pay..Is there law that requires psysicans to send itemized bill. Who knows please send me some links

They wouldnt itemize it submit it to collections which say will take me to the court for $278 I owe them. WHy they wouldnt itemize it?

There will be a level of service code and possibly a procedure code. The level of service is determined by how complex the medical decision making is for the case.
