What is the person's responsibility to correct the problem(s) if the following has occured?:?


1) Before birth the parents agree to name their child Klara Ann Wilson (first middle last).
2) Notification of Birth Registration from the hospital reads "Klara Wilson" (first last).
3) Transcript of records of both primary & secondary education reads "WILSON CLARA" (notice the name is in all uppercase letters with no comma in between).
4) Correction of Birth Certificate at age 6 reads "Clara A. Wilson"
5) US Passport issued at age 6 but after the Correction of Birth Certificate reads "Clara Ann Wilson".
6) Primary & secondary educations' diplomas read "Clara A. Wilson", despite the transcript of records reading "WILSON CLARA".
7) State ID, Social Security card, & professional license all reads CLARA A WILSON (notice there is no period after A, & all letters are in uppercase).
8) Now an adult, Ms. Wilson decides to amend the name on the US Passport to Clara A. Wilson (the name on the birth certificate). Somehow it was denied.
9) And the medical records reads Klara Wilson.

As Ms Wilson is now an adult it is her responsibility to make any changes.

She should go down to the courthouse and apply for a name change, once the documentation is in place for the official name change she can apply for new ID and passport by providing certified copies of the court name change document.
