Im looking for a form for child custody transfer from mother to grandparents.?

I need a form for the insurance company that says Myself and my husband have custody of our grandson. So he can be insured thru our policy. Any help?

If you legally have custody you should have documents to support this. ie. court or adoption papers. If the case is that you are not legal guardians but have custody you will need to consult a lawyer.

Unless the child's mother has sole legal and residential custody and the child's father has no rights (rom the court) for the child, she cannot assign guardianship to you without the court or His permission.

Therefore, she needs to file a petition for guardianship and serve the child's father.

Unless the proper procedures are followed you all will be committing fraud.
