What kind of paperwork is issued when from an adoption...in the 1960's?


If you had any experience at all with something like this i would appreciate your help. I really kinda need to know this. thanx.

my older sister was adopted in the 60's and I have old paper work from her adoption. I do not know what it was called but it was a form stating that she would be from that point on known as ---her name---- plus her adoptive parents last name..
